The John McGivney School Authority provides for a smooth transition for students entering and leaving the John McGivney School Authority School Program. The John McGivney School Authority recognizes that services to families must address the multiple changing and intertwining needs of children, families, and community service providers. The following strategies have been implemented to assist families making decisions at crucial crossroads of their child’s educational and rehabilitative needs. The premise underpinning coordination of services is based on the recognition that pediatric rehabilitation services can be overwhelming for families. Consequently, clear, defined plans must be articulated.
For children receiving JMCC therapy services, who are school-aged and met the eligibility criteria for admission to the John McGivney School, the following processes occur, with parent consent:
- Referral (From JMCC Therapist)
- Collation of Assessments
- All appropriate data including medical and other assessments are procured.
- Multidisciplinary Team Meeting
- Family, rehabilitation team and educators meet to determine the suitability of the program and student’s needs.
- A Release of Information Form is completed.
- School Visit
- Student and family visit school and meet staff.
- Initial Registration
